We have a cabover camper we have used for the last 14 years.
It is getting harder for my wife to climb into the cabover and ever harder to get down in the morning.
We are thinking of a small fifth wheel.
However, we will be keeping the camper.
I have looked at the underbed mount hitch(leaving a flush bed). Basically a removable ball in the bed and an adapter to convert to a fifth wheel.B&W makes the unit.
Has anyone used this type of arrangement?

check the TC forum also as those guys use their trucks for other purposes.

I had an arctic fax 1140 TC and it fit fine in my Chevy 3500 with the Reese Signature series removable 5th wheel hitch.

My 3500 truck has the service body.
I have about 1/2" clearance between the camper and the box so nothing can be above the bed.

How about just a removable 5th wheel hitch? I have a Reese and whith the 5er hitch removed, the two mounting bars are very low. Haven't measured them, but they might be slightly higher than your 1/2" requirement.

I use the B&W hitch with the companion, and like it very much. Takes about 15 minutes to pull the companion out and use the goose neck or use the flat floor. When you hook up the companion, you know that it is latched by looking at the jaws around the pin. Four way swivel head. The price is reasonable, in my view. I have used mine for almost seven years now and no problems.

Why not just go to a small TT. They are what might fit your needs.
