Does anyone know where the RV-Waiver form can be downloaded from? Directv insists that the only way to get the form is to have it mailed to my home address. Thanks.

Found this on the DirectTV FAQ when I searched for "RV"
Do I need waivers if I want DNS in my RV?
Federal legislation allows the delivery of DNS service to RVs and commercial trucks without the need for waivers, if the satellite dish is permanently attached to the vehicle. Customers must provide a copy of their vehicle registration and a Mobile Vehicle Declaration of Intent in order to qualify for this service. Because of the way that 'RV' is defined under this law, boats do not qualify for this exception. Other limitations may also apply. are you sure you want one? if you have Direct in your home they might try to nail you for second service in RV. don't ask, don't tell unless absolutely necessary.

There is no requirement that the dish be permently mounted on the RV. At least, if there is, it is not enforced in my experience as I take my dish off of the house and mount it on a tri-pod when I travel and just send in the wavier about two weeks prior to leaving home.
There was some discussion awhile back with DirecTv CS that if I traveled with one receiver in the RV I could not get service on a second receiver left at home. This was never an issue for me as I take both with me when traveling so I never tested if this was enforced.

the permanent mount is another don't ask and don't tell.
